Head to a local Social Security office and fill out a replacement card application. Or download the proper forms via the Social Security Administration website.

Be sure to have identification. If you choose to get a replacement Social Security card in person, have your drivers license, state issued I.D., passport, school I.D., birth certificate or other acceptable form of identification.

Opt to mail in your Social Security replacement application. If you do not need a new copy of your Social Security card immediately, you can mail in the application with certified copies of proper identification.

Avoid losing your Social Security card. To prevent lost, stolen or destroyed Social Security cards, do not carry it around with you. Keep the card in a safe place and only bring it with you when you need to show the card for official business.

Carry a plastic or metal card. If you must carry a copy of your Security Card, or forget your Social Security number easily, carry a metal or plastic version that won't easily be destroyed. These cards are rarely acceptable forms of identification; they are more for your own use.
